This form is for West One Student Accommodation tenants to report maintenance required on their property. Please provide full information below and we will be in touch as soon as possible.

COVID19: we will call before attending, so if a shared household please nominate a housemate in case you are not available

Are you waiting for the results of a COVID19 test?

At present/in the past 14 days, do you show any of the following symptoms:

A high temperature?

A new, continuous cough (3 or more episodes over 24 hours)?

A loss or change to your sense of smell or taste?

At present are you, or someone in your household, self-isolating?

Have you ever been diagnosed with Covid-19?

Have you travelled overseas (or come from overseas) in the past 14 days?

Have you been contacted by the Track and Trace system in the last 14 days?
